The following is a confidential Hospital and Shut-in visitation list for Nazareth Evangelical Lutheran Church. This database may only be accessed by the Spiritual Care Committee and the Pastor. If you are not on the Spiritual Care Committee and have made a visit to a member of the church, please call or e-mail someone on the Spiritual Care Committee or Pastor Voss so this information may be updated.

Guide to the spreadsheet:
Name – Name of individual
Location – Location of individual (home, hospital, etc.)
Visitation Record – the last five visitations of this individual including date, initials of the person who visited, and if they received Communion.
Example: PV, 10/21, C = This means that Pastor Voss (PV) visited on
October 21 (10/21), and gave them Communion (C).
Latest Non-Confidential Status – the latest status of this person. Only share information that is non-confidential, such as expected release date or any special requests. Please initial and date all entries (i.e. DV, 10/21)
Background Notes – information that help us learn more about the background of this person. Please initial and date all entries (i.e. DV, 10/21)
